First year Students Mandatory Health Forms
To find and submit health forms, please visit the “Forms” section on the BearCare Portal at
Required forms:
Due July 30th for non-Athletes
This form asks for historical health information, (which can be self-completed), and contains a PDF Physical Form (including immunization record) to download to bring to your medical provider to complete. Once the Physical Form has been completed by your provider, you will upload a copy to this Personal Health Record and submit. You cannot submit without the Physical Form.
Due JUNE 30th for ALL students
This form is a TWO-STEP process.
First, you will see a link to an outside web address. This site allows you to waive or enroll in Ursinus’ health insurance plan. If you have your own health insurance plan, you will waive this coverage and enter in your plan’s information. You will receive approval or denial of your waiver based on the information provided, so please answer carefully!
If you do not complete the waiver successfully, you will be automatically enrolled in the insurance and charged for the year on your tuition bill. Charges begin on July 1, 2025, but can be be removed if the waiver is completed by September 1, 2025.
Second, you will then return to the form on the BearCare Portal and enter your current insurance information to be stored on our system in case of emergency.
Student Athletes
Student Athletes will be required to complete additional forms. They will also be found on the BearCare Portal under “Forms.”
